The Conference is planned for 2 full days. It will be held on 25 and 26 February 2011. The Inaugural Asian Conference on Integrated Care 2011 focus on bringing new ideas, concepts and technologies in the practice of integrated care by the physicians and health care professionals. The purpose of this conference is to raise the integration across all health care professionals globally. The importance of integration is to bring out better health outcomes.
The key strategy:
- Achieving continuity of care over time.
- Bringing together not just services at different levels (i.e. vertical integration of acute hospitals with primary care and intermediate and long- term care (ILTC) providers), but also across different horizontal services for one-stop provision of care where appropriate, such as packagingcomprehensive home care services for the elderly to include preventive, medical, nursing, rehabilitative as well as psychosocial care.
- Combining health with social services such as housing and transport for a more holistic and efficient care plan.
- Developing platforms for integrated policy-making and management.
The Conference Objectives:
- To introduce concepts of care integration and transitional care, and how do they begin to measure its cost-effectiveness.
- To introduce the concepts of population health, and care assessments.
- To highlight innovative transitional and community programmes across regional and international countries, and how they are adapted for local implementation.
- To develop quality frameworks and showcase quality initiatives related to the intermediate and long term-care (ILTC) sector.
- To understand the different models of supporting and sustaining patients in the community.
